A few things to remember:
- Patients who have well visits scheduled during the above flu clinic times will receive their shot then. Your other children who do not have scheduled visits will need to be scheduled at one of the flu clinics to receive their flu vaccine.
- Flu shots are for Cloverleaf Pediatrics patients only.
- Print, complete and bring the Influenza Questionnaire (PDF) to your flu shot appointment.
- Flu shots require active, eligible insurance on the date of service. We will bill your insurance company, but you will be responsible for any portions that go to deductible, co-insurance or are considered non-covered.
