Please take some time to acquaint yourself with some basic policies of Cloverleaf Pediatrics.
Vaccine policy
As board certified pediatricians, we firmly believe and exercise preventive medical care for all our patients and the children in the surrounding communities. For this reason, we require all patients to be fully immunized against vaccine-preventable illnesses. More information about these preventable diseases is available from the American Academy of Pediatrics.
Our practice vaccinates according to guidelines set forth by the American Academy of Pediatrics and the Centers for Disease Control & Prevention.
Appointment policies
If you are late for a scheduled appointment, it may be necessary for us to reschedule your appointment due to the volume of patients scheduled to be seen. This policy ensures each scheduled patient has sufficient time with his or her pediatrician.
We require a 24-hour notice for cancellation of appointments. If the office is not contacted 24 hours prior to the appointment you will be billed a $25.00 No Show fee.
After three missed appointments without a call to cancel in advance, we reserve the right to increase the No Show fee to $50.00 or to terminate the physician/patient relationship.
We realize that unforeseeable circumstances such as illness or accidents may occur shortly before your appointment. Such instances will be reviewed on a case-by-case basis.
Your financial responsibilities
You are responsible for providing us with the correct insurance information at every visit.
You are responsible for selecting the correct physician as the PCP for select plans; not doing so may result in personal liability for the bill.
Understanding your insurance coverage
You are solely responsible for educating yourself about your insurance.
Understanding deductibles, services not covered by your plan and co-insurance rules will save you money and prevent both the stress of unexpected bills and repeated requests from your insurance company for information before they remit their payment.
Please note that payment plans are always an option if you are unable to pay your balance all at once. If you would like to set up arrangements please contact our billing department. Self-pay patient visits are paid the same day of service. For details please, contact our office staff at 636-939-3362.
